First and the easiest way to get the Blackened Femur is by completing the Whispers of the Dead in World Tier 3 and World Tier 4. However, it is not guaranteed that you will get a Blackened Femur as this task can reward you with any of the four Malignant Body Parts.

One of the easiest ways to gather the Malignant Body Parts is to hunt the Grotesque Debtors. Defeating these enemies will drop a random Malignant Body Part for you. So if you don’t get a Blackened Femur from one, make sure to go for other Grotesque Debtors, as it will only increase your chances of getting the desired item.
