Follow the northwestern shore until you see the two shell icons pop up on your minimap. The first one is along the beach and the other is quite close to it, though a bit further inland.

Continue following the shoreline going to the northeast and you’ll spot this as well.

Go across the shallow body of water to reach the smaller landmass northeast of Broken Isle. This Phantasmal Conch is right next to a Silver Pocketwatch memento.
