Let’s talk about the Stone Tablet Fragment in the Catacombs zone.

Fast travel to the Great Hall respite and go left. Battle the mobs here until you reach the top-left section.

Open the locked door and go through it.

There’s a pustulant monstrosity here and several flies (and their spawning nest). After you eliminate the mobs, check the altar to find the item.
