The Flask of Wondrous Physick in Elden Ring can be found in the Third Church of Marika. This location is in Mistwood, a small area along the borders of Limgrave and Caelid. You can pick up an additional Sacred Tear for your regular flasks, and it’s where Melina pays you a visit as well.
With the Flask of Wondrous Physick in your possession, you’ll notice another command once you interact with a Site of Grace. This lets you make different combinations of Tears (at least two) so that you can gain both effects. Just remember that the Flask of Wondrous Physick only has a single use, and you have to rest in a Site of Grace to replenish it.

Most of the Tears used by the Flask of Wondrous Physick in Elden Ring will come from Minor Erdtrees in various regions. Simply find that region’s map and head to the Erdtree icon to reach it.
