How to get and use Gurgling Head in Diablo 4

One of four Malignant Body Parts.

Many Diablo 4 players have been wondering about how to get and use the Gurgling Head item, which is a special item required for summoning a certain boss.

If you’re struggling to find the Gurgling Head or don’t know what to do with it, we’ll explain everything. It’s not a common item to find, but we can point you in the right direction.

Diablo 4: How to get and use Gurgling Head

If you’re looking to fight the Echo of Varshan, an Uber boss, you’ll need the Gurgling Head, alongside three other Malignant Body Parts. There are a few different ways you can come across a Gurgling Head, such as caches from Whispers of the Dead on World Tier 3/4, turning other Malignant Body Parts into it, or a drop from a Grotesque Debtor.

First off, you must be on either World Tier 3 or 4 to even find Malignant Body Parts. You’ll find them more often inside random caches you receive from completing Whispers of the Dead. The four parts required to summon the Echo of Varshan are the Gurgling Head, Blackened Femur, Trembling Hand, and Malignant Heart.

Note: You only need the Malignant Heart if you want to summon the Echo of Varshan on World Tier 4.

You may also find Grotesque Debtors who are rare elite enemies that can drop Malignant Body Parts upon defeat. During Season 2, you used to be able to spot them in Blood Harvests, but with that Season over, you’ll have to search throughout areas where you’re completing Grim Favors. Look out for Grotesque Debtors as you make your way through Grim Favors for the Tree of Whispers.

One final way you can get a Gurgling Head is by turning other Malignant Body Parts into it. Visit an Alchemist and they’ll be able to create a Gurgling Head using these materials:

  • Malignant Heart
  • x4 Abstruse Sigils
  • x15 Veiled Crystal

You’ll find Malignant Hearts through the same methods that you get the Gurgling Head, which is by killing Grotesque Debtors or completing Whispers of the Dead. Abstruse Sigils are available from salvaged rings and amulets, while Veiled Crystals are found from salvaged Rare and Legendary items.

Once you have a Gurgling Head, alongside the three other Malignant Body Parts, you can summon the Echo of Varshan in the Malignant Burrow, which is inside The Writhing Mire beside the Tree of Whispers.

Now you can go ahead and fight the Echo of Varshan for special drops like Unique items or the Mucus-Slick Egg, which is used to summon another Uber boss named Duriel.
